We are a Class C-non-ambulatory licensed CBRF or Community Based Residential Care Facility. Our designation allows us to care for all residents including those who are totally dependent. Dementia Support Group of The Alzheimer’s & Dementia Alliance of Dane County - We are a lead facility with the Dementia Support Group, successful in the care of advanced dementia/alzheimer’s residents in our community needing intensive and often one-on-one support. We continuously offer extensive training on the special needs of our residents.
Care Wisconsin
We have been chosen as a 2014-2015 Quality Advisory Group Board Member and pilot facility for a new Program of Excellence rewarding those facilities who go beyond their contract requirements in order to provide quality care above & beyond for Care Wisconsin residents statewide.
Workforce Development Board & Madison College
We have also been recognized in the community by the Workforce Development Board for our partnership in their programs designed to help, develop and train young people and searching individuals in our communities. We have also been asked to join several quality advisory boards to assist agencies in our community and state to better understand the needs of our residents, providers, and how to best assure quality care.

UW Hospital – Member of the Assisted living Acute Care Transitions Coalition:
Working with area hospitals – particularly UW Hospital – to coordinate quality care and quality transitions for our residents who require hospital treatment in order that hospital experience continues the quality individualized health care that our service care plan provides to maintain overall health stability outside our homes.
UW Hospital – Respite Contract
We have a long-standing Respite Contract for patients needing continued health care support outside the hospital in a less restrictive/acute care setting prior to complete hospital discharge.
Nursing Home Relocation Program
We are an experienced quality facility for this welcome program, offering a long-term alternative to many who have other options for long term care that were formerly unavailable or unfamiliar to them. Fountainhead Homes have become home to many from nursing homes releasing a new joy on life
