Our Team

The Fountainhead Homes Family


Since 2000, Fountainhead Homes has been supporting residents with complex & combination diagnoses such as Alzheimer’s & Dementia, Diabetes, Parkinson’s, Chronic & Progressive Diseases,  and Mental Illness. What we have learned is that outside of the details of their diagnoses, all residents can best thrive in an environment that is intensely focused on their individual needs.

Our staff is certified in medication management and additionally trained in the special medical/physical needs of our residents – for example; sliding scale insulin, G-tubes, wound care, catheters, injections, lift support, feedings, and aphasia. Under the supervision of our RN, management team, and outside trainers, our highly trained staff members are able to use their knowledge and skills to provide a “home for life” for each of our residents, even as health care needs change or intensify. We offer palliative care and work with a hospice partner as needed. Our goal is to let residents stay at “home” with our Fountainhead family.

Our RN, Director, and Administrator are available onsite on a regular basis to get to know residents and proactively coordinate best care practices with staff. This familiarity with our residents makes proactive responses to changes in their needs – whether emotional or physical – a large factor in the quality reputation we have earned since 2000.

Fountainhead Homes is a leader in the care of residents with advanced dementia. While dementia can follow similar progressions, we have found that changes can often be varied and challenging. Our smaller, more intimate atmosphere is ideal for implementing a tailored plan of care. Residents with dementia need to be able to move at their own pace, express themselves freely, and most importantly: be heard. We’re able to build relationships of trust with our residents because they feel our compassion and desire to understand them. This enables us to be creative in providing best care practices. Our strong relationship with Dr. Kim Peterson and the Alzheimer’s & Dementia Alliance gives us additional support and guidance to meet our standard of healthcare excellence.

All staff members at Fountainhead Homes are experienced caregivers who must meet our company’s high expectations, including a depth of compassion that mirrors our mission. All staff must sign our Mission Care Agreement confirming their understanding of who we are and who we serve.

Dianna, RN

Dianna Carlson has been a nurse for over thirty years.  She has worked in hospital medical/surgical units, oncology, home health and assisted living facilities. Although she has worked in a variety of settings over the years working at Fountainhead has fit her like a glove. Dianna trains staff, oversees the medication administration and regularly assesses residents to help prevent and maintain their health concerns. She is dedicated to the staff, residents and their families.

Dianna is married with two children. Her daughter, Erin, is a third year pharmacy student at the University of Wisconsin. Dianna’s son, Ryan, is a senior at the University of Minnesota with a major in aerospace engineering.

Dianna enjoys her 1979 VW convertible and attending antique car shows with her husband, Rob.

Mate is a familiar face to Fountainhead Homes’ residents and health care partners. For nine years she had been working at our Ashwabay House as a Lead Caregiver until her character, charisma, & capabilities naturally led her to assume our Director position in 2012. Mate brought with her extensive experience working various healthcare positions at other locations, but she ultimately committed herself  to the mission and the residents at Fountainhead Homes. She has  ALWAYS been a resident and family favorite because of her unwavering dedication and tenderness. Health partners rely on Mate for facilitating complex changes to care & medication routines.

Mate was born and raised in her native country Zambia and came to the United States in 2002.  Mate expanded her family getting married in 2013, and has also welcomed a beautiful daughter, Kendra.  Boy, can Kendra light up our Houses!!

Joe and Ruth Polacek, Owners

Joe and Ruth’s union was unlikely, coming from different parts of the country; but their mutual interests were the links that led to more than a marriage. Joe spent many years in and out of school doing missionary work and community outreach within the United States, and also performed direct care in a management capacity for complex health settings. Despite other professional and self-owned business experiences, the draw to connect with “others in need” remained paramount. Ruth completed her studies and began a path of business and management in a variety of settings which led to an understanding of many state and federally funded Programs. Ruth had family in the healthcare field who exposed her to complex physical and mental health diagnoses at an early age. Thus, the union of Joe and Ruth personally led to a very natural opening of their first Fountainhead Home, caring for 13 residents in 2000.

“We were honored to have the privilege to serve our residents in such an intimate setting, allowing us to KNOW our residents and their families so well. We had found what we had both been looking for – a place where our children could grow, bond and learn, and where our family could connect to gifted staff who shared the same mission to serve. Our family is now Fountainhead Homes.”

IMG_8095 (2)“Hello, my name is Luther. I am the youngest in the Fountainhead Family. I am also the quietest, most polite, and the busiest. I’m also quite large, but most people don’t really notice this about me. All of my friends live at Fountainhead Homes. We like to sit together. I’ll sit next to them or on their lap. I also sniff them, rub noses with them, or if they are in bed that day, I put my head on their lap or chest so they can smile at me. Once they get a chance to scratch my ears or my back, I lay down on the floor by their feet. If you come by for a visit when I am there, don’t let my busy routine stop you from coming over and showing me some affection. A good scratch can serve as an introduction, and I like anything with peanut butter.”
