Fountainhead Homes is intimate and tailored care at its finest. Because the details matter, we are committed to serving no more than 8 residents in each of our homes. Our locations are beautifully designed, supremely comfortable, and offer virtually unlimited opportunities for the personalization of resident living spaces.

We welcome residents at all stages of their healthcare journeys. Our low staff-to-resident ratio makes the transition to Fountainhead Homes smooth and comfortable. Additionally, our experienced nursing staff actively participates in all facets of resident care and develops a close rapport with physicians and healthcare partners. From the day our residents arrive, they are pampered with care. And that care is never “one-size-fits-all.” We get to know the personal needs and preferences of each of our residents and then deliver the highest level of care based on that understanding. We find that we learn new things from our residents every day, and this enriches all of our lives.

  • We are a Class C-non-ambulatory licensed Community-Based Residential Care Facility. Our designation allows us to care for all residents including those who are totally dependent. We are pleased to offer a welcome nursing home alternative!

    Our management team is uniquely certified and qualified to provide a program of care for our residents with complex health and emotional needs, including:

    • Advanced age

    • Physically disabled

    • Alzheimer’s/Dementia

    • Emotional Disturbance/Mental Illness

    • Traumatic brain injury

    • Chronic illness

  • Good food can bring joy and contribute to a sense of community. At Fountainhead Homes, we make every effort to fulfill that need. We are proud to serve delicious home-cooked meals and desserts on a daily basis. We refrain from using “prepared” meals, and try to support desires for favorite items as much as possible. Our dietary program includes:

    • Three well-balanced and dietician-approved meals provided daily

    • Snacks & drinks provided

    • Groceries delivered weekly for freshness

    • Great care and respect for residents’ individual dietary restrictions and tastes
